Health Information Privacy Statement
Ontario has legislation to protect the privacy of your personal health
information. The Personal Health Information Protection Act, 2004, also
known as PHIPA, has been in place since November 1, 2004. As a Registry
under PHIPA, CCN is authorized to collect, use and disclose (to those
authorized) personal health information about you and other cardiac patients
in order to facilitate and improve cardiac care in Ontario.
Collection of Personal Health Information
As a Registry under PHIPA, CCN collects personal health information about
cardiac care patients in Ontario from 18 cardiac care hospitals. For
example, the personal health information collected may include your name,
date of birth, address, health history and information collected during
cardiac centre visits.
Uses and Disclosures of Personal Health Information
As a Registry under PHIPA, CCN uses and discloses personal health
information for different activities. For example:

Assisting cardiac care providers to coordinate access to care for cardiac
patients
- Managing data for analysis
- Reporting information about Ontario's cardiac care system and provide advice
about cardiac care planning to the Ministry of Health and Long-Term Care
- Important Information About Privacy of Personal Health Information
CCN takes steps to protect your personal health information from theft and
loss, as well as unauthorized access, copying, modification, use, disclosure
and disposal.
We conduct audits and complete investigations to monitor and manage our
privacy compliance.
We have a privacy policy that guides all CCN activities. We take steps to
ensure that staff and everyone who performs services for CCN understand and
apply our privacy policy.
